Metabolic Health · Insulin Resistance
Your blood sugar is “normal.” Nobody checked your insulin.
The weight settled around your middle and will not move. The 2 p.m. crash is a daily event. The cravings run the show. And you were told to eat less, move more, and that it is probably your age. Here is what usually got missed: your blood sugar can read “normal” for years while your body works overtime to keep it there. That overtime has a name. It is measurable. And almost nobody looks for it early.

Finding 01 · what is actually happening
Insulin resistance, in plain English
Insulin is the hormone that moves sugar out of your blood and into your cells for energy. With insulin resistance, your cells stop listening well, so your pancreas shouts louder — it pumps out more and more insulin to get the same job done. For a while, that works. Your blood sugar stays in range. But you are now running on high insulin all day, and high insulin is what tells your body to store fat, especially around the middle, and to hold onto it no matter how hard you try.
So the exhaustion, the cravings, the belly weight that ignores every diet — those are not a willpower problem. They are what running on high insulin feels like.
Finding 02 · why it hides
Why a “normal” result kept you in the dark
A standard visit checks fasting glucose, and maybe HbA1c. Both are real tests. Here is the catch: they measure the sugar, not the effort. Your glucose is the last thing to break. It can sit at a tidy 95 for years while your insulin quietly climbs to keep it there — and then one day it is not tidy anymore, and you are told you are “suddenly” prediabetic. It was never sudden. Nobody was measuring the part that changes first.
Finding 03 · the markers that show it early
What a fuller look actually reads
When it is appropriate for your situation, a more complete metabolic review looks past a single glucose to the pattern underneath:
- Fasting insulin — the number that moves years before glucose does, and the one standard panels almost never order
- HOMA-IR — your fasting insulin and glucose read together, a plain estimate of how hard your body is working
- Triglyceride-to-HDL ratio — a quiet flag for insulin resistance hiding inside a “normal” cholesterol panel
- HbA1c and fasting glucose — your longer and shorter-term blood sugar, read as context, not the whole story
One number in isolation is easy to wave off. Read together, next to how you actually feel, they turn a “normal” result into something you can finally act on. If you want the fuller picture, that starts on the metabolic overview.
Patient intake · what people describe
Does this sound like you?
- Weight that settled around the middle and will not budge
- An afternoon crash that hits at the same time every day
- Cravings for sugar or carbs that feel impossible to out-willpower
- Feeling shaky, foggy, or short-fused when a meal runs late
- Doing “everything right” and watching the scale ignore you
None of these prove insulin resistance on their own. Together, they are a reason to look properly — not to be told, one more time, to just try harder.

Addenda · insulin resistance & the Clarity Visit
Can I have insulin resistance if my blood sugar is normal?
Yes — that is the whole point. Your body can keep glucose in range for years by running insulin high. A normal glucose does not rule it out; it just means no one measured the part that moves first.
What is the one test that gets missed?
Fasting insulin. It is inexpensive and it changes early, yet a standard visit rarely orders it. Read alongside your glucose (as HOMA-IR) and your triglyceride-to-HDL ratio, it tells a much fuller story.
I have tried every diet. Why would this be different?
Because the goal here is not another diet. It is understanding what your body is doing and why your effort has not paid off — then a plan built for that, sequenced for a real week. The content here is educational and is not medical advice or a diagnosis.
Is this available outside Pennsylvania?
Clinical services are limited to adults physically located in Pennsylvania at the time of care.
One next step
Bring the “normal” labs and the weight that won’t move.
Another year of “just try harder” is another year of crashes and cravings while the part that changes first goes unmeasured. The sooner it’s read, the sooner it can change.
